Drew County Social Security Number Applications

I've been getting SS-5 forms (Social Security Number applications) for some of my ancestors and relatives, and thought I would share the data here.

SS-5s are a great alternative to birth certificates for folks born before Arkansas started keeping records. The person applying for a Social Security Number gave the information himself or herself, which gives a higher probability of its being correct.

If you have SS-5s you have collected, please feel free to share them on the mailing list.

I don't believe the Social Security Administration will even send you these records without proof that the individual is deceased. I print all my request letters from the Social Security Death Index (SSDI); that way I'm sure that even the SSA knows these folks are deceased.

Both Ancestry.com and Rootsweb.com have Social Security Death Indexes which let you print out a form letter to request specific SS-5s for your ancestors or relatives. I like Rootsweb's treatment of the SSDI better because it also lets you attach "web post-it notes" to records you're interested in. That way, when others come along and view the same records, you can make contact with fellow researchers and cousins.

Go to http://ssdi.rootsweb.com or http://www.ancestry.com/search/rectype/vital/ssdi/main.htm. You can do simple or advanced searches on both sites. Once you've found a record you're interested in, click the link to generate the SS-5 request letter and print it.

Unfortunately, the SSA recently raised its fees from $7.00 per request to $27.00 per request, when the Social Security number is known (if unknown, the fee for a search and copy is $29.00. I don't know if the $29 is refunded if no record is found). There also seems to be an option called NUMIDENT which costs $16.00. This appears to be a computerized printout of the information contained on the SS-5 form. If you order the SS-5 itself, you will get an exact reproduction of the form, filled out by hand, of the person requesting a Social Security number. (I have never ordered the NUMIDENT option, so don't know if it contains all information on the SS-5. The applicant's signature must be missing from this computer printout, but I don't know what else is missing.)
